Rhinoplasty Surgery

Rhinoplasty is a surgical intervention that aims to modify the size, shape, or placement of the nose and its different parts for medical or aesthetic purposes.

An intervention that must be performed by a plastic surgeon expert in nose operations and must be carried out in a medical center that has all the guarantees. Otherwise, you can expose your health to great risks.

Unlike what happens in other aesthetic operations, it is recommended not to undergo a rhinoplasty before reaching the age of 16, since it is important to wait at the end of the physical development of the candidate or the candidate to achieve the desired results.

As medical professionals often remember, in the case of rhinoplasty performed on minors, it is important that the decision to have a nose surgery be agreed between the patient and the parents. Also that both are advised by the surgeon when defining the shape, placement, or size of the new nose.

As for the intervention, it usually lasts between one and two hours depending on the complexity of the operation. The patient will undergo a local anesthetic with sedation or general anesthesia, depending on the complexity of the operation.

Once the anesthesia is applied to the patient, the nose is separated from the bone and the cartilage to later sculpt the shape of the septum and its surroundings according to what was agreed in the preoperative period.

Once the shape of the nose is modified, the skin will be replaced and the opening sutured. It is important to note that the incisions that are usually made in the pits are minimal, so there will be no scars after the operation.

Types of rhinoplasty techniques:

Rhinoplasty can be carried out through different techniques that allow you to modify the shape, placement or position of the nose, either as a result of a malformation at birth, an accident, following the appearance of certain respiratory problems or simply for aesthetic reasons, as it happens most of the time.

Once the specific case of the patient has been examined, it is the rhinoplasty specialist who, in agreement with the patient, is responsible for increasing or decreasing the size of the tip or bridge, removing the easel, widening or narrowing the shape of this or of doing the same with the fins. The different techniques that can be used are described below.

Traditional rhinoplasty

This intervention aims to improve the patient's facial appearance by totally or partially modifying the nose, respecting the characteristics of the intervened face in search of a natural result previously agreed upon.

Traditional rhinoplasty can be carried out in two ways: either open or closed. If it is open, it is intervened externally, while on the later it is operated internally, which prevents the appearance of scars. In both cases the intervention usually lasts between 60 and 120 minutes.

Septoplasty (Deviation of the nasal septum)

This nose operation is performed to correct the nasal septum and the deviation of the cartilage wall and the bone that divides the nostrils. Normally these are operations that are aimed at improving the breathing of patients, although they also serve to correct possible internal bleeding or pain in this area.

In septoplasties, the septum is repositioned through an incision that improves the entry and exit of air through the nasal passages. These are usually carried out under local or general anesthesia depending on the complexity of the intervention not exceeding 90 minutes.

Profileoplasty (Rhinoplasty + Mentoplasty)

The application of two surgical techniques to harmonize the shapes and lines of the patient's face from the nose to the chin is known as profiling.

The most common, as reported by the various associations of aesthetic doctors, is to combine rhinoplasty with chin modification, although more and more patients are taking advantage of anesthesia to expand the touch-ups to other areas such as the forehead, lips in search of more harmony in the face.

It is usual that for the mentoplasty (intervention to correct the chin), the remaining tissue of the rhinoplasty of the patient is used to develop an implant that subsequently, after making a minimal incision under the chin, helps to shape the chin.

In addition to this type of rhinoplasty, it is important to emphasize that there are different treatments such as those that incorporate hyaluronic acids or hydroxyapatite that, through infiltration, allow a slight modification of the shape of the tip of the nose with temporary results that require a new intervention every two years.

How Does It Work?

Step 1: Anesthesia

Medications are administered for your comfort during the surgical procedure

The doctor will recommend the best choice for you

There are several choices for anesthesia during rhinoplasty. These include general anesthesia or intravenous sedation

Make sure that the surgeon and anesthesiologist have a complete list of all the medications you take, and that they are aware of all your pertinent health information

Step 2: The incision

Rhinoplasty is performed either using a closed or open procedure

In an open rhinoplasty, the incisions are made across the columella, the strip of tissue that separates the nostrils. This procedure is generally considered more invasive and is reserved for major surgeries.

A closed rhinoplasty involves hidden incisions inside the nose and is used for smaller adjustments to the shape of the nose. Through these incisions, the skin that covers the nasal bones and cartilage is raised, allowing access to shape the structure of the nose

Through these incisions, the skin that covers the nasal bones and cartilages is gently raised, allowing access to reshape the structure of the nose

Step 3:  Reshape the nose structure

Once the incisions have been made, the doctor can perform the process of reshaping the nose in accordance with what was discussed with the patient prior to the surgery

An overly large nose may be reduced by removing bone or cartilage. Sometimes surgery of the nose may require the addition of cartilage grafts

Most commonly, cartilage from the septum, the partition in the middle of the nose, is used for this purpose. Occasionally cartilage from the ear or rarely a section of rib cartilage can be used.

Step 4: Closing the incision (s):

Once the underlying structure of the nose is sculpted to the desired shape, nasal skin and tissue are redraped and incisions are closed.

Additional incisions may be placed in the natural creases of the nostrils to alter their size

A splint is usually applied to keep the nose in place as it heals and to protect it from accidental trauma, some patients might also have internal splints or tubes as well

Some patients remain in the hospital overnight for observation, while most patients are allowed to go home after surgery within a few hours

MedicW Insider tips

The preoperative or preparation for the nose operation is very simple. As with most aesthetic interventions, it is only necessary to undergo a blood test to confirm that the rhinoplasty candidate is able to get into an operating room and receive the relevant anesthesia to carry out the operation.

The day of the intervention should go to the fasting hospital, as in the majority of the surgeries. In the event that you are a regular smoker or consume alcohol on a regular basis, it is important to notify the surgeon as you may be asked to refrain from these habits during the hours before the operation.

Rhinoplasty is a simple aesthetic operation, so there are hardly any complications after rhinoplasty and, when they appear, they are minimal and related to the inflammation and pain that this usually causes in the first hours after the operation.

Although it is not frequent, some patients have reported the appearance of small red spots on the skin, which is due to the rupture of some blood vessels that will disappear usually after several hours.

When the rhinoplasty is performed by a specialized surgeon, the results obtained tend to last for the rest of your life

Results

Once the surgery is finished, the patient should be really careful for the first 48 hours, as any impact on your nose could generate complications. From then on the patient should be able to return to their daily routines. It is usually recommended to wait for a week until going back to work, although it would also be possible to return somewhat earlier, this should be determined on a case by case basis.

Doctors usually recommend for the majority of the aesthetic interventions, to take special care during the first weeks after the intervention. Something that we develop in the next paragraph and that is essential to achieve perfect and lasting results.

Postoperative

Among the recommended care during the postoperative period of rhinoplasty, in addition to avoiding significant bumps or scratches in the intervened area, avoid sun exposure during the first two months and not exercise during the first three weeks. Avoid tobacco and alcohol-based on the recommendation indicated by the medical team.

Other aspects to review the postoperative period have to do with the aesthetic issue, since after the intervention of the nose, a plaster is placed on the intervened area, in addition to plugging the two nostrils to prevent bleeding and give stability to the nasal septum .

A discomfort could only last a couple of days. During the first 72 hours, you should take the indicated anti-inflammatory and analgesic drugs to minimize the inflammation and bruising of the operation you have undergone.

In the most severe cases, these discomforts last up to a maximum of two weeks, the plaster will not remain placed on the nose beyond 10 or 15 days. The usual thing, however, is that the postoperative period be easy and bearable.

In any case, it is important that you always follow the instructions provided by the surgeon's medical team that intervened you to shorten the postoperative as much as possible and make it as bearable and painless as possible.

Finally, remember that, in case of any doubt related to the rhinoplasty, preoperative, postoperative, medication or any technical aspect related to the nose operation, it is advisable to contact the medical center either in person, by phone or through of the mail. It will be there where the patient care team offers answers to all your questions or questions.
