Breast Augmentation Surgery

Breast Augmentation Surgery

The breast augmentation surgery or mammoplasty surgery is a surgical intervention that is used to increase the size of the breasts, as well as modify their shape and reposition them wherever each patient wants.

In addition to altering the size and silhouette of the breasts, the size and position of both the areola and the nipple can also be modified in the same mammoplasty procedure. Modifications that affect specific areas of the breasts, which can be carried out without the need to increase or decrease the size of the breasts.

There are currently two types of techniques to achieve breast augmentation. The first and most common is the one that goes through making incisions under the breast, around the areola, or in the axillary area to introduce the breast prosthesis of the size desired by the patient. The area through which the prosthesis will be inserted will vary depending on the type of prosthesis, the size of the prosthesis, as well as the technique used by the mammoplasty surgeon.

The second, which is gaining popularity, is the one that achieves the increase of the breasts by extracting and repositioning fat of the patient herself. During this procedure, fat is removed from an area that is accumulated and then injected into the breasts. The amount of fat that will be used in the operation will vary depending on the increase desired by each patient.

Types of prostheses:

There are different types of breast prostheses. These can be classified according to the filling, the cohesion of the gel that composes them, or the form it acquires.

For example, depending on the filling we find, on the one hand, silicone implants, which are the most recommended since they prevent spilling of the filling in case of breakage, in addition to achieving a natural appearance, both insight, and touch. In addition to these, there are also saline serum implants, which are those made with physiological serum. Being composed of a solution of water and salt, in case of breakage, the body can absorb it completely naturally. Its touch is also the most natural, although slightly harder than silicone.

In addition to their filling, these can also be divided according to the cohesion of the gel they use. It is here that we find those of medium cohesion, somewhat softer to the touch and those of ultra cohesive silicone gel, harder and harder to deform that require a somewhat more invasive operation

Finally, we can also classify them according to their shape by finding round breast implants, the most used worldwide or anatomical or with an oval shape that allows them to achieve much more natural shapes and attract less attention.

Once the type of implant to be placed and the technique to be used to carry out the breast implant are determined, the patient will be summoned to undergo mammoplasty. You will be informed in detail of how the operation will be, as well as the type of incision that will be made to place the implant and how it will evolve.

There are several types of incisions that can be carried out to place breast implants.

The most common is the inframammary incision, in which a small path is opened below the breast from which the prosthesis is inserted. You can also apply a periareolar incision in which an opening is made in the diameter of the areola to later introduce the prosthesis through that area. Finally, it is also possible to make a trans axillary incision to insert the prosthesis through the armpit, which helps to hide the scar.

Usually, the surgeon will recommend one based on the patient. Already in the operating room, after the incision, it will be time to insert the implant submuscular or on the mammary muscle. The area where the implant will be placed will depend, again, on the type of prosthesis, the size of the prosthesis, its physique, and the considerations that the surgeon can make in relation to your specific case.

Once the implant is placed, the incision will be closed either through a few stitches, skin adhesive or surgical tape. It will be then when the postoperative period begins that we can offer more information later.

Surgery duration: between one to two hours

Duration of hospital stay: You might go home the same day as the surgery

How Does It Work?

Step 1: Surgery preparation

At the arrival of the medical facility

You will change into the hospital gown

Most likely, the surgeon will use a marker to indicate where the breast implant incision will be

The area of the surgery will be scrubbed with an antimicrobial soap to minimize the risk of infection

Step 2: Anesthesia

Medications are administered for your comfort during the surgical procedure.

The doctor will recommend the best choice for you

There are several choices for anesthesia during breast implant surgery. These include general anesthesia, conscious sedation and local anesthesia

Make sure that the surgeon and anesthesiologist have a complete list of all the medications you take, and that they are aware of all your pertinent health information

Step 3: The incision

Incisions are made in inconspicuous areas to minimize visible scarring

You and your plastic surgeon will discuss which incision options are appropriate for your desired outcome

Incision options include: along the areolar edge (peri-areolar incision), the fold under the breast (inframammary fold) and in the armpit (axillary incision)

Incisions vary based on the type of breast implant, degree of enlargement desired, your particular anatomy and patient-surgeon preference

Step 4: Inserting and placing the breast implant

After the incision is made, a breast implant is inserted into a pocket

If you opted for silicone-filled implants, the already-filled implants will be put in place; saline breast implants are usually filled after the shell is put in the pocket

If you have very small breasts, or if you are having implants inserted after a mastectomy (surgical removal of one or both breasts), you may need a tissue expander inserted for a period of time before receiving your permanent breast implant(s)

After the breast implants are in place, your surgeon will check for symmetry. He or she will place you in an upright position to see how the implants look when you are seated

Step 5: Closing the incisions

Incisions are closed with layered sutures in the breast tissue and with sutures, skin adhesive or surgical tape to close the skin.

Over time the incision lines will fade

The quality of scar depends on many things, including your genetics, exposure of your body to nicotine and infection

MedicW Insider tips

The preoperative mammoplasty is simple. Beyond performing an analysis to confirm that your state of health is optimal to face a surgical intervention that requires general anesthesia and to ask you to go to the clinic fasting on the day of the operation, you do not have to make any special preparation to undergo a breast augmentation operation.

You should know that it is possible for the surgeon or his medical team to ask you to comply with other guidelines like stop smoking or drink alcohol, at least on the day before the surgery. Something that will be informed in the pre-operation visits.

Since it is a minimally invasive aesthetic surgery, the recovery should be relatively fast. However, it is possible that during the first week or ten days you feel some discomfort that could be addressed by taking some anti-inflammatories that will be prescribed when the time comes.

As for the use of the protective bra, it must be worn only during the first weeks after the intervention. During this period it will only be withdrawn for revisions and cures that will take place in the first hours after the aesthetic operation. Your doctor will inform you about the specific length.

Once the protective bra can be removed and the doctors inform you that everything is "OK" you can live a normal life without worrying about anything. It is important that you know that once you get rid of the protective bra and the prescribed medication, you should still wait several weeks to observe the final results.

Although after three months you can start to see the definitive results, some doctors advise that it can take up to 6 months, in addition to seeing the breast augmentation process completed, to fully appreciate how the patient feels with her new breast.

It should be noted that, if you opt for a clinic specializing in cosmetic surgery that has experienced mammoplasty surgeons, the results obtained could be better.

Finally, it should be noted that it is possible to undergo future breast interventions in the event that you want to increase your breasts again or alter your shape and also so that, if desired, you can decrease their size or modify your shape or the placement of both the breasts and the areolar area.

The postoperative period is fast compared to most cosmetic interventions. So much so that the day after the intervention it is possible to leave the medical facilities and begin to recover normally. Of course, it is very important to follow, one by one, each and every one of the recommendations made by the medical team that has intervened and will do the follow-ups after the surgery.

It is essential that you know that during the first days after being operated it is normal to feel some discomfort due to the inflammation of the intervened area. Also that these discomforts will gradually decrease until they disappear completely usually after a couple of weeks.

After a couple of weeks, the only discomfort you should feel is that of having to get used to new anatomy.

And, thanks to both scientific and technical progress, mammoplasties have improved greatly in recent years, achieving much shorter, safer, more precise operations that require much shorter recovery times than were needed just a few years ago.
